Relationship-based care is our methodology to enhance the delivery of patient care, treatment and service and support the behaviors associated with The Caring Model TM at Presbyterian Hospital of Denton.  Our goals include creating and sustaining:

Unit practice councils are comprised of a mixture of staff from each individual unit and department, meet regularly to monitor progress and set direction for the group. Unit practice councils examine how their work is accomplished and care/services are delivered. Questions include:

·        How are decisions made?

·        How is work allocated or assigned?

·        What is communication like for your department?

·        How is care managed and delivered?

·        How do you, department staff, create a caring, feeling environment in your job function?

·        What financial and human resources are required to

assure that competencies are maintained?

 

Relationship-based care is about:

·        Clarity, Competency, Confidence, Collaboration, Communication,

           Contribution, Commitment, Caring, Connection
