Presbyterian Hospital of Denton has numerous patient safety initiatives in our efforts to provide a wide array of healthcare services to the community we serve.

Take an active role in documenting and maintaining an accurate, up to date listing of your allergy and medication history. This will assist health care professionals who care for you to have the most current information possible.

Every effort is taken by the hospital staff to obtain an accurate listing of medications. Spend a few minutes completing this form, noting all medications (prescription, over the counter, herbal and nutritional supplements) that you are currently taking.

Keep this form in your wallet at all times and review it with your physician and/or pharmacist when you see them. Make sure you update the information whenever there is a change.
