Pre-Registration
We want your registration process to be as convenient as possible. Our friendly and helpful registration desk representatives are happy to help you if you have any questions or concerns regarding your registration and admission.
Pre-Registration Options
We ask that you pre-register once your Physician has scheduled your surgery, procedure or test.
You may only pre-register through our online application or through central scheduling at least 24 hours prior to your surgery, procedure or test.
Patients who complete this form still need to go to the regristration/admissions area to finalize your registration.

On The Day of Your Surgery, Procedure or Test
We ask that you arrive at least 2 hours prior to your surgery, procedure or test.
In order to make sure your registration process goes smoothly, please have the following information ready when you register:
- Current insurance card
- Picture ID
- Any orders/prescriptions the Physician may have given you
Upon your arrival, you will be asked to:
- Sign in at the front desk
- You will then be assigned a beeper that will let you know when the next available Registration representative is ready to see you.
- The Registration representative will ask you to verify your current information and sign consent forms, etc.
- You may be required to pay a co-payment during this time. The amount of co-pay varies and is based on the amount your insurance will cover. Please refer to the Patient Billing FAQs for any questions.
